If you’ve been researching breast augmentation recently, you may have come across the term “Preserve” (Preservé), particularly on social media, where it has attracted significant attention overseas. The technique has now arrived in Australia, alongside the introduction of the latest generation of Motiva implants, and I’ve recently completed formal training in it.
Because much of what patients are seeing online originates from international marketing, I wanted to write a straightforward explanation of what the Preserve technique actually is, how it differs from conventional breast augmentation and, importantly, who it may and may not suit.
What Is the Preserve Technique?
The Preserve technique is a tissue-preserving approach to breast augmentation developed in collaboration with Motiva, the manufacturer of a range of silicone gel breast implants.
In conventional breast augmentation, the pocket that holds the implant is created by surgical dissection, carefully dividing tissue to make a precisely shaped space. The Preserve technique takes a different approach to pocket creation, using purpose-designed instruments:
- Inframammary incision: A short incision is made in the inframammary fold, the natural crease beneath the breast.
- Channel creation: A specialised instrument called a channel separator is used to develop a narrow pathway through the tissue plane.
- Pocket development: An inflatable balloon is introduced and gradually expanded to develop the implant pocket. Rather than dividing the natural ligaments that support the breast, this graduated expansion is designed to work around them, preserving the breast’s own supporting architecture.
- Implant insertion: The Motiva implant is delivered through an insertion sleeve using a no-touch technique, meaning the implant is not handled directly during placement.
The implant is positioned above the chest muscle, and the incision is typically shorter than that used in conventional breast augmentation.
Why Has This Technique Been Developed?
The design intent behind the Preserve technique is to reduce tissue disruption during surgery. The breast has a network of ligaments that contribute to its natural shape and support. Conventional pocket dissection divides some of these structures; the Preserve technique aims to preserve them, with the goal of maintaining the breast’s natural support and reducing trauma to surrounding tissue and nerves.
It also reflects a broader shift in what many patients are seeking from breast augmentation. In recent years, there has been a clear trend towards more modest implant sizes and subtle, proportionate results rather than dramatic changes in volume. The Preserve technique was developed with this style of augmentation in mind.
Who May the Preserve Technique Suit?
This is where I want to be direct, because it is the part most often glossed over online.
The Preserve technique is not a universal replacement for conventional breast augmentation. It is a specific tool with specific parameters.
It Works Within a Defined Implant Size Range
The technique is designed for use with particular Motiva implant types within a defined range of sizes. Individuals seeking a larger augmentation may be better served by a conventional approach.
The Implant Sits Above the Muscle
Where individual anatomy, such as very limited natural breast tissue, makes placement under or partially under the muscle more clinically appropriate, a different approach may be discussed.
It Does Not Reshape the Breast
The technique places an implant within the breast’s existing architecture; it does not alter the breast itself. Where there are changes in breast position (ptosis), significant asymmetry or anatomical variations affecting breast shape, procedures such as mastopexy (breast lift), with or without implants, may be more appropriate and can be discussed during consultation.
In my view, appropriate patient selection is the single most important factor in whether any surgical technique delivers a good outcome. That assessment can only be made through consultation and clinical examination.
An Honest Note on the Evidence
The Preserve technique is a newer surgical approach, and long-term outcome data continues to be collected. Early published data is encouraging with respect to implant stability, but as with any newer technique, longer-term results across broader patient populations will take time to accumulate. I discuss this openly with patients as part of informed decision-making, alongside the risks that apply to all breast augmentation surgery.
Breast augmentation using the Preserve technique remains surgery. It is performed under general anaesthesia in accredited private hospitals, and it carries the risks associated with breast implant surgery, which are discussed in detail during consultation.
